Diabetes PATH Toolkit

Designed to help individuals actively manage diabetes and build confidence in everyday self-care, this evidence based program is open to adults of all ages, includes those with prediabetes, type 1 or type 2 diabetes, family members and caregivers. Participants will learn practical strategies for managing symptoms, making informed treatment decisions, and setting achievable health goals.
